Dawn Marie (Collinsworth) Farley, 51, of Xenia, went Home to be with her Lord and Savior, Jesus Christ, on Sunday morning, June 28, 2015, at Hospice of Dayton. She was born May 11, 1964, in Dayton, Ohio, the daughter of Richard A. and Beverly A. (Coon) Collinsworth. She was a member of AHOP, a 1982 graduate of Xenia High School and the University of Kentucky, and was a past president of the UK Alumni Club. She was employed by Premier Health Care. She is survived by her husband, Christopher O. Farley, whom she married, June 20, 1987, her mother, Beverly A. (Rodney) Collinsworth Grimes, her mother-in-law, Martha E. Farley, a son, Dr. Kyle C. (Dr. Emily J.) Farley, two daughters, Lauren E. Farley and Caroline G. Farley, a sister, Julie J. (Chato) Denton, a sister-in-law, Candy (Todd) Maynard, a brother-in-law, Jeffrey Farley, aunt, Betty J. (Gorbachan) Singh, nieces, Britney M. Elofskey, Samantha Denton, Abby Farley and Jessica Maynard, nephews, Brandon Elofskey, Shane C. Denton, and Quentin Farley, a great-nephew, Aiden C. Biestrek-Elofskey, numerous cousins, by many close family and friends, including special friends, the DeFarCott's. She was preceded in death by her father, Richard A. Collinsworth, maternal grandparents, Norman and Sylvia Coon, paternal grandparents, Herman and Francis Collinsworth, and by her father-in-law, William Farley. Services will be held at 10am, Monday, July 6, at AHOP, 282 Stelton Rd., Xenia, with Pastor Greg Delaney officiating. Visitation will be held from 3-7 pm, Sunday, at the church. Burial will be in Woodland Cemetery, Xenia. In lieu of flowers, contributions may be made to Freedom Church (AHOP) or to the University of Kentucky Alumni Association, 400 Rose St., Lexington, KY 40506, in her memory.
